Symphony’s Care Plan App, Care Team, and service coordination comes at a fixed monthly fee based on the level of care needed and within the first six months we are able to complete the initial comprehensive assessment and establish continuity with the best service providers.

Symphony comes to the home to assess not only the health and medical needs, but also the details in the residence, financial and life-care planning, and what family and social needs are crucial to maintain a healthy sustainable independence at home. This important first step is the foundation for building a custom care plan unique to each client’s needs.
Health Profile: Symphony provides a thorough assessment of your health including an assessment of limitations in Activities of Daily Living (ADL/IADL), chronic conditions and treatment, cognitive tests (SLUMS), mood (PHQ-9) and activities.
Medical Plan Profile: We review current Medicare and supplemental insurance plans, current medications and drug plan, provider plans for ongoing care, and assess eligibility for other plans and coverage.
Residential Profile: Symphony evaluates utilities, home and yard maintenance conditions, equipment functionality, fall risk factors and mobility conditions, accessibility to grocery shopping and meal prep; pet care; transportation teams; home nursing, palliative, dementia, and hospice care.
Financial Profile: We review the integrity of routine financial & legal needs, such as life-care planning, power of attorney documents, medical claim reconciliation, automated on-line bill-paying, credit card monitoring, budgeting and prevention of fraud, and coordination with professional advisors to consolidate important documents into the Care Plan App.
Family & Social Profile: Symphony assists in organizing and documenting personal and family information, family decision-making roles, Powers of Attorney (POAs, MPOAs), Advanced Directives (ADs), social needs and activity schedule. Our Care Plan App keeps everyone up-to-date with the built in calendar feature.
Communication is Everything
Symphony uses our Care Plan App, a cloud based application to ensure that a senior’s trusted care team, relatives, and advisors have around the clock access to the latest conditions, medications, scheduled appointments, any tasks that need attention, and contact information for all on the individual’s care team. This comprehensive communication helps reduce anxiety, confusion, and costly mistakes while giving loved ones peace of mind.
Orchestrated Care at One Predictable Cost
We believe that our clients benefit by knowing that our services come at a predictable, fixed monthly rate. Rather than charging hourly fees of $100-125, Symphony spreads the costs of all of its services over an initial six-month period at a fixed rate per month. Base rates vary according to client needs and circumstances, but generally range between $500 and $1,000 per month for our full-time care-coordination services and Care Plan App. Some of our clients need additional, a la carte, services for special circumstances, more frequent home visits or other services, which are priced separately. Our care partners bill the client directly for their services or third-party insurance as appropriate. If, following the initial assessment, a client does not wish to pursue care coordination with Symphony, the client will simply pay a $500 fee for Symphony’s comprehensive assessment which the client may keep. If you choose to continue with Symphony, the assessment is included in the monthly fee.
